Sorry to hear about the loss of your pregnancy at four months. Technically your doctors are right, it is a miscarriage. I know that for a woman who truly wants a child that each pregnancy loss is really a loss of a baby.

There are several reasons why a woman might have contractions at the time of her pregnancy loss. the first is that contractions are the way the uterus puches out whatever is inside it. If your pregnancy stopped growing for any reason, you would most commonly have contractions when the uterus expelled the fetus.
Sometimes other tyoes of conditions can cause the uterus to contract and push out an otherwise viable fetus, a uterine septum or other congenital uterine abnormality, or a large intrauterine fibroid are examples. The best way to evaluate your uterus is with a hysterosalpingogram or hysterosaline examination.

Irregular menstrual periods often mean irregular ovulation. The estrogen shot may help your bleeding but it won't help you to ovulate.

You need to be evaluated by a reproductive endocrinologist who can evaluate the various issues that you have raised.
