A very large varicocele can cuase testicular atrohpy. That would explain why there is no sperm now, when there was 'some' a few years ago. Being that you conceived a few years ago, does not mean that there where a lot of sperm available then, it was a 'miracle' that was not realized.

You did not indicate when the surgery was. We had a very similar situation, with a large varicocele, no sperm, and surgery...it took well over 6 months till we finally saw some sperm. So don't be devestated yet!

I hope you did the microsurgery with a specialist, as very large varicoceles, corrected in this manner, have good success rates in general.

Also, keep in mind, that though it may never become good enough for 'natural' conception, you can be pretty at ease that your husband can 'produce' sperm, and that one way or another, you will, god willing have a child!
