As others have noted, I'm doubtful that any responsible RE is going to help you "increase your chances for having twins." There are significant medical risks associated with carrying twins both for the mother (particularly with advanced maternal age) and the fetuses. However, as you know, women's fertility declines sharply after 40 so you may well be a candidate for fertility medications or ART. In any case, if you are interested in having another child, you should make an appointment with an RE who can discuss your particular circumstances with you. Ellen Bender, AIA Board
