I go to Mt. Sinai/NYU, and their policy is to attempt to go to blast for all women under under 37. HOWEVER, if its not appropriate in your particular case, i.e., you are a poor responder, you don't have enough embryos, they will do a three-day transfer. I know because I am 36, and I did a three day transfer because I had a low response and poor fertilization. While the 5-day transfer is the default, they obviously are still considering what is appropriate in the individual case.
