There are several ways to treat PCO patients for infertility. Clomid is one of the simplest and easiest to administer. How many cycles of Clomid to do is a good question and varies by patient depending on many factors including age, semen analysis.... Most treatments have the highest efficacy in the first three attempts (coupled with ovulation).
Low dose treatment protocols include the use of very low amounts of hormone treatment injections callled gonadotropins. These are given daily and the person is watched carefully for her response as there is an increased risk of an exaggerated response producing multiple follicles thereby increasing the odds of a multiple pregnancy or hyperstimulation. When to switch is dependent on both physician and the patient and requires a thorough discussion about the merits of both. Good luck.
